Request for Evaluation Form
The
Office of the University Registrar is responsible for collecting, tabulating, and reporting on course and faculty evaluations. Evaluation results are used in decisionmaking for rank and tenure, merit review, and are available to the university community.

Final class lists and Request for Evaluation forms distributed by the University Registrar to departments. |
| 9th week of semester |
Request for Evaluation forms due to the Office of the University Registrar by close of business. If evaluations are requested for TAs, please include them in the request. Qatar faculty may request evaluation forms here. |
| 11th week of semester |
Evaluation forms distributed by the University Registrar to departments. Please verify that you have received the forms you requested. |

Evaluations due on the day of the last class meeting. Instructors are encouraged to distribute evaluations at the last class meeting and not before. |
Fall - 1st week of February
Spring - 1st week of July
Summer - Last week September

Evaluation results reported to departments, dean's offices, the Office of Institutional Research, the University Rank and Tenure Committee, the Georgetown University Student Association, and posted online. Information disclosed to the above varies. Please contact the University Registrar for more information. |
Guidelines
To ensure proper collection, tabulation, and reporting, please:
- Use only forms specially printed for the course and faculty member;
- Remind students to use pencil on forms;
- Allow students to complete forms in silence.
Confidentiality
Only aggregated data is released to students. The results of some questions (I-5; II-2, 3, 5, 6; III-2, 3, 5) are sent to the Georgetown University Student Association for online posting unless requested otherwise on the original request form. If the faculty member indicated to opt-out of the published results, the label on the evaluation envelope is marked with three asterisks. If the faculty member changes their mind or the University Registrar is mistaken, please notify us.
